Generic comment action

A generic comment action should just write a comment into the change log for the entity. Comment actions should be available on:

• addresses
• subunits
• locations
• streets
• street_names

Street names currently do not have a change log, like the others. We may need to implement one. Otherwise the Names::comment action could just change the notes field on the street name.
