Add documentation for exchanges between soil carbon, nitrogen and phosphorus pools

Is your feature request related to a problem? Please describe.
At the moment, (as @vgro pointed out in #593) the soil documentation doesn't properly explain how nutrients get exchanged between pools. This is largely because this is something that I'm still working on and don't have a fixed structure for yet

Describe the solution you'd like
Once I have settled on a structure for these exchanges this section of the documentation needs to be reworked and extended.
