Fractions in indicators

Many of the indicators are fractions where whole numbers would be expected, e.g. Causes of Death. This is probably due to the remapping of geographical boundaries. I think that this should be called out explicitly next to each indicator otherwise it is impossible to know what methodology was used to calculate the numbers
