annotation: new register entry form

• upon saving attempt the form is cleared, which is rather unfortunate because a. if this failes (e.g. due to not being logged in) one must re-enter the entire form and b. there's no way to check again and re-edit the register entry. I think it would be better if the form stayed open and had an explicit "Close" button.

• save button for the new register entry is only available on the top bar of the form, repeating it close to the bottom would be also convenient (as well as providing a keyboard shortcut for the save entry action)

• cancel button when adding new entry (when user attempts to add a new register entry but then changes mind)
