Add Survey Banner to Website

To gather feedback and understand our users' needs, we're introducing a user survey. The goal is to embed a survey banner on the Mathesar website to facilitate feedback collection, allowing us to improve our product based on user insights.

• The banner should be fixed at the top of the website.
• It should be visible on all pages of the website
• Provide users an option to close the banner. Once closed, the banner should not reappear for that user during the same browsing session.
• The banner should contain a direct link to the provided "Quick User Check-in" survey form.
