Patient Charting Database Project

This project is a Patient Charting Database for Evergreen Health Clinic, designed to manage and streamline patient information, clinic operations, and medical records using a web-based interface.

Citation All code is based on the CS 340 starter code provided by Oregon State University, with the exception of specific modules and functionalities described below:

• The handling of many-to-many relationships (Patients-Doctors, Patients-Diagnoses) is entirely our own work.
• Custom form validations and additional error handling mechanisms are original contributions.
• The user interface enhancements, including dropdown menus and CSS styling, are unique to this project.
